New Intake/Consultation
Initial Phone Consultation

Child/Adolescent
Initial intake is a 90 minute appointment that is split with the client and the parent. An additional follow-up appointment may be needed to complete the intake depending on the complexity of the case.
Adult
Initial intake is a 60 minute appointment.
Follow-Up Visits
Duration of 20, 30, or 50 minutes is based on complexity and nature of follow up.
Additional services may require additional fees that are based either on an hourly rate or a set fee.
Payment
In order to provide individualized care,
Dr. Chakkaravarthi does not accept or participate with any insurance provider and is considered an out-of-network provider at Reliance Psychiatry, PC.

All payment is due in full at the time of the service.

Payments are accepted via debit card, credit card, or HSA/FSA card.
​
We require a credit card or debit card to be kept on file via our secure electronic medical record system/payment system while in treatment.
​
We can provide you with a super-bill at your request that can be submitted to your insurance provider or for your HSA/FSA plans.
Your insurance provider may reimburse you for a portion of your fees, but we strongly encourage you to discuss these matters with your insurance provider ahead of time.

Office hours may change at certain times of the year or expand in the future.
Clients with any psychiatric or medical emergencies should immediately call 911 or go to the nearest emergency department.
